We need all information 2 business days before your appointment. If we don’t have all information in a timely manner, it will delay your appointment.

The following forms are needed, prior to your first appointment:

  1. New Client Intake Packet
  2. Financial Agreement
  3. HPPGA Credit Card Authorization
  4. Copy of client driver’s license – send photocopy to our email or fax.
  5. If using insurance, copy of insurance card (front and back)

Sliding Scale Agreement

Third party payor – HPPGA Credit Card Auth. (If someone else is paying for sessions of an adult client) –
