The following forms are needed, prior to your first appointment:
- New Client Agreement Packet
- HPPGA Credit Card Authorization
- Third party payor – HPPGA Credit Card Authorization (If someone else is paying for sessions of an adult client) –
Use this if someone else will pay for your sessions. Have them also send photo id. - Copy of client driver’s license – send photocopy to our email or fax.
- If using insurance, copy of insurance card (front and back)
