The following forms are needed, prior to your first appointment:
- New Client Intake Packet
- Financial Agreement
- HPPGA Credit Card Authorization
- Copy of client driver’s license – send photocopy to our email or fax.
- If using insurance, copy of insurance card (front and back)

Third party payor – HPPGA Credit Card Auth. (If someone else is paying for sessions of an adult client) –
