Client Forms & Information

We need all information 48 business hours before your appointment. If we don’t have all information in a timely manner, it will delay your appointment.

Makes the process simple!

The following forms are needed, prior to your first appointment:

  1. New Client Agreement Packet
  2. HPPGA Credit Card Authorization
  3. Third party payor – HPPGA Credit Card Authorization (If someone else is paying for sessions of an adult client) –
    Use this if someone else will pay for your sessions. Have them also send photo id.
  4. Copy of client driver’s license – send photocopy to our email or fax.
  5. If using insurance, copy of insurance card (front and back)
